Creating SOAP Notes From Transcriptions With AI

A SOAP note is an invaluable tool for documenting and communicating patient information. Atlas.md automatically generates AI-generated SOAP Notes from your audio files, providing you with structured, ready-to-review documentation directly in the patient’s chart.

When you upload an audio file, Atlas.md transcribes it in the background and instantly generates AI output based on the type of recording:

• Doctor–patient conversations produce an AI SOAP Note
• Other audio types (like staff updates or voicemails) produce an AI Summary

Once processing is complete, you’ll see an AI SOAP Note button appear next to the audio file. Click it to open the note and review the AI-generated content. From there, you can decide how to store or refine it based on your workflow.

When you’re ready to save the note, click Promote to Chart Note.

From here, you can take several actions. Click:

• Add Note to store the note as-is
• Edit AI Results to make adjustments before saving
• Copy to Clipboard if you’d like to paste it elsewhere

Once saved, the SOAP note becomes a top-level chart entry and remains linked to the original audio and transcript for easy reference.

If you prefer to generate or refine a summary manually, you can still do that anytime:

• Click the AI button on the right side of the patient’s chart, then select Generate SOAP Note
• In the AI Actions pop-up, review the content, choose the desired AI action, and click Process with AI

• Once you’re satisfied, select Add Note to create a top-level chart note, or Add to Thread to keep it attached to the original audio entry
• To make changes before saving, click Edit AI Results

Your summarized conversation will always appear in the same thread as the audio file and its transcript for easy reference.
