There are a couple ways you can add a consent form to a patient profile.
- When the patient enrolls online, you can add the consent form during that process. You can set this up from your clinic settings. Click Account > Settings > Patient Enrollment Forms > View and Update Settings > Upload New Enrollment Agreement.
- You can have the patient sign a physical form, scan the document, then upload it to your patient’s profile as an attachment. You can also include a note when you upload it this way.
